1. FY20 PCHP Supplement Progress Report Overview

A subset of Fiscal Year (FY) 2020 Ending the HIV Epidemic - Primary Care HIV Prevention (PCHP) award recipients requested and were awarded additional funding from FY 2023 to further expand HIV prevention services that decrease the risk of HIV transmission. This award provides a one-year performance period during which time award recipients will submit two semi-annual progress reports in 2024. The PCHP award’s objectives are to increase patients tested for HIV, increase patients prescribed PrEP and/or increase timely linkage to care (within 30 days) for those newly diagnosed with HIV as part of the Ending the HIV Epidemic in the U.S. initiative.

2. Accessing the FY20 PCHP Supplement Progress Report

1. Login to EHBs.

2. From the EHBs home page, click the Tasks tab to view the Pending Tasks - List page.

3. The FY20 PCHP Supplement Progress Report task will show 'Progress Report' in the 'Task' column and will show your H80 grant number in the 'Entity' column.

4. Click the Start link in the 'Options' column, to begin your progress report. 

3. Completing the FY20 PCHP Supplement Progress Report

1. On the Progress Report - Status Overview page, you will see the applicable forms to complete the FY20 PCHP Supplement Progress Report. The report consists of two forms:

  • Contact Information
  • FY20PCHP Supplement Form

2. Click the Update link in the 'Options' column next to the Contact Information Form.

3.1 Completing the Contact Information Form

1. Enter the contact information for the point of contact for your FY20 PCHP Supplement progress report.

2. Click the Save and Continue button to proceed to the FY20PCHP Supplement Form.

3.2 Completing the FY20 PCHP Supplement Form

1. Complete the following questions on the FY20 PCHP Supplement Form.

Question 1: Project Status

  • Provide an approximate status for your project.
  • The "Fully Implemented" option will be disabled until the final FY20 PCHP Supplement progress report.

Question 2: Please provide a status update and describe successes/promising practices and lessons learned on each activity proposed in your Primary Care HIV Prevention (PCHP) Supplement application. Identify what activities have been fully implemented, partially implemented, and/or not yet started with this funding.

  • For questions 2a-2d, select "Fully Implemented", "Partially Implemented", or "Not Yet Started" for each activity supported with PCHP funding.
  • Provide a written status update for each activity proposed in your approved workplan that has a response.

Question 3: Are the implemented or planned activities described above and associated uses of the funds consistent with your approved PCHP award submission?

  • If no, make appropriate selections.

If Option 3.1 or 3.2 apply to you, select only one. The two budget reallocation options cannot occur at the same time.

If you have a budget modification that affects the funded categories, submit a ticket through the BPHC Contact Form to discuss with a PCHP Investment Team member.

Note: Option 3.3 Equipment will not be available for selection as equipment is not an approved use of PCHP grants.

Question 4: Are there or do you anticipate any issues or barriers in the use of the funding and/or implementing the planned activities consistent with your approved PCHP work plan submission?

  • If yes, Question 4.1 and 4.2 will appear for you to make appropriate selections for any barriers encountered and any steps taken to address them.

  • You may attach documents to provide additional context for any of the above questions.

2. Click the Save and Continue button to proceed to the Review page.

4.Submitting the FY20 PCHP Supplement Progress Report

1. On the Review page, review your progress report forms to ensure that all information entered is accurate. Access each form by clicking on the View link in the 'Options' column. Click on the Continue button to proceed to the Submit Report page.

2. On the Submit Report page, click on the Submit button to submit your progress report to HRSA.

5.Responding to Change Requests for the FY20 PCHP Supplement Progress Report

1. After submitting the FY20 PCHP Supplement Progress Report, HRSA may determine that additional information or corrections are needed to what was originally submitted. 

  • In this case, an EHB email notification with more details regarding the requested changes will be sent to you.
2. If a change request is sent, your progress report will be accessible to you immediately in your pending tasks queue. Follow the steps found in section 1. Accessing the FY20 PCHP Supplement Progress Report to revisit and update the information in your progress report consistent with the email notification.
