Healthy Start

Form Overview

On the Healthy Start forms, the grantee must provide details on participants who received interpersonal violence screening and referrals, prenatal care, perinatal/postpartum care, well-woman visits/preventive health visits, and depression screening and referrals. These forms also collect data on infants placed to sleep following safe sleep guidelines, infants ever breastfed or breastfed at 6 months of age, child participants receiving well-child visits, participants with health insurance coverage, prenatal tobacco and e-cigarette use, low birthweight infants, infants born preterm, and infant deaths to enrolled HS participants to calculate infant mortality rate.

NOTE: If HS 04 form was selected in V2-2022 OMB package, then Healthy Start form will be selected by default in V3-2023 OMB package but will not have data pre-populated. The existing HS forms of DGIS system HS 01, HS 02, HS 03, HS 05, HS 06, HS 07, HS 08 and HS 09 will be deleted.

The form will display under Healthy Start Forms section on the left menu when the grantee start/edit a DGIS performance report.

Pre-population

The following data will pre-populate from one report to the next report: 

  • Data entered into the Annual Performance Objectives section.


Note: Data will be entered into the NCPR and pre-populate to subsequent NCCPRs and PPERs. Data in this section will not pre-populate between V2-2022 OMB reports to V3-2023 OMB reports.

Form Sections

The Healthy Start form has the Annual Performance Objectives section and the following two tabs that collect data for various measures:

    • Healthy Start Benchmarks (HS 4, 10-18)
    • Healthy Start Outcomes (HS 19-21)

Section I. Annual Performance Objectives

  • In this section, the grantee provides objectives for New Competing Performance Reports (NCPRs). Every objective field for each measure will be required and validation will display on the fields: Enter an integer: 0 – 100.
  • For Non-Competing Continuation Performance Reports (NCCPRs) and Project Period End Reports (PPERs):
    • Pre-population: All budget period objectives will be prepopulated from the previously approved report.
    • Annual Performance Objectives: For current and previous years, annual objective fields will not be editable.
  • For Non-Competing Continuation Performance Reports (NCCPRs) future years, the grantee will have the ability to edit/modify the annual objectives fields, if needed.
  • For Non-Competing Continuation Performance Reports (NCCPRs) and Project Period End Reports (PPERs) the grantee provides data in the two tabs.

Healthy Start Benchmarks (HS 4, 10-18) Tab

In this tab, the grantee may expand the accordion to edit the forms HS 4, 10 -18. All measures under this tab will be collapsed and grantee can expand the accordion to view or enter the details.

HS 04 - Interpersonal Violence Screening and Referral

HS 04 measure calculates the percentage of HS women participants who received interpersonal violence screening and referral.

Section II. Data Collection for Detail Sheet - Interpersonal Violence - Screening and Referral

In this section, the grantee must provide data for the current budget period for NCCPRs and PPERs.

HS women participants screened for IPV using a standardized screening tool

In this section, the grantee answers the following questions:  

The Numerator (Number of HS women participants who received interpersonal violence screening using a standardized screening tool during the reporting period.)

  • The grantee can enter integer values 0 - 999,999 and the following validation will display on the data entry box: Enter an integer: 0 - 999,999.

NOTE: This numerator value must be greater than or equal to the Denominator (Number of HS women participants who screened positive for IPV during the reporting period.)

Denominator (Total number of HS women participants in the reporting period.)

  • The grantee can enter integer values 0 - 999,999 and the following validation will display on the data entry box: Enter an integer: 0 - 999,999.
  • The denominator value must be greater than the numerator value and the following validation will display on the data entry box if the entered values do not meet the criteria.

The Denominator must be greater than or equal to Numerator.

In this section, the grantee answers the following questions:  

Numerator (Number of HS women participants who screened positive for IPV during the reporting period and received a subsequent referral for follow-up services.)

  • The grantee can enter integer values 0 - 999,999 and the following validation will display on the data entry box: Enter an integer: 0 - 999,999.

The Denominator (Number of HS women participants who screened positive for IPV during the reporting period.)

  • The grantee can enter integer values 0 - 999,999 and the following validation will display on the data entry box: Enter an integer: 0 - 999,999.
  • The denominator value must be greater than Numerator (Number of HS women participants who screened positive for IPV during the reporting period and received a subsequent referral for follow-up services.) value and the following validation will display on the data entry box if the entered values do not meet the criteria.

The Denominator must be greater than or equal to Numerator.

  • This denominator value must be less than or equal to the Numerator (Number of HS women participants who received interpersonal violence screening using a standardized screening tool during the reporting period.) and following validation will display on the integer box:

The Denominator for referral (2nd sub-measure) should be less than or equal to the numerator for screening (1st sub-measure)

This section will be automatically populated based on the information provided in Section II. Data Collection for Detail Sheet - Interpersonal Violence - Screening and Referral.

  • A table will display two auto calculated percentage fields for current reporting period as follows:

HS 10 measure calculates the percentage of pregnant HS participants who receive prenatal care beginning in the first trimester.

Section II. Data Collection for Detail Sheet - Prenatal Care

In this section, the grantee must provide data for the current budget period for NCCPRs and PPERs.

Numerator (Number of pregnant HS participants who began prenatal care in the first trimester of pregnancy. Refer to Detail Sheet for definition.)

  • The grantee can enter integer values 0 - 999,999 and the following validation will display on the data entry box: Enter an integer: 0 – 999,999.
NOTE: The number of pregnant participants is unduplicated. Pregnant HS participants should be counted only once during a calendar year unless they have experienced more than one pregnancy in that calendar year. (If pregnant participant is counted more than once in the numerator, they should be counted more than once in the denominator.)

Denominator (Total number of HS women participants in the reporting period.)

  • The grantee can enter integer values 0 - 999,999 and the following validation will display on the data entry box: Enter an integer: 0 – 999,999.
  • The denominator value must be greater than the numerator value and the following validation will display on the data entry box if the entered values do not meet the criteria.

Denominator must be greater than or equal to Numerator.

Section III. Annual Performance Score - Prenatal Care

This section will be automatically populated based on the information provided in Section II. Data Collection for Detail Sheet - Prenatal Care.

Section II. Data Collection for Detail Sheet - Perinatal/Postpartum Care

In this section, the grantee must provide data for the current budget period for NCCPRs and PPERs.

Numerator (The number of pregnant/newly postpartum HS participants, who had enrolled prenatally or within 30 days after delivery, and received a comprehensive postpartum visit within 12 weeks after delivery. Refer to Detail Sheet for definition.)

  • The grantee can enter integer values 0 - 999,999 and the following validation will display on the data entry box: Enter an integer: 0 - 999,999.
NOTE: The number of pregnant/newly postpartum participants with a postpartum visit within 12 weeks of delivery is unduplicated.
