OGH Performance Reports  

This help page provides step-by-step instructions for Office of Global Health (OGH) grantees on how to complete Performance Reports, specifically created by the OGH Performance Module. 

The steps on this page will walk you through the different types of fields you will see on the performance report and some common tips on completing this form.

Performance Module Form

In the EHBs, OGH grantees can access their Performance Report submission tasks in the Grant Submissions section. From your EHBs homepage, go to the Tasks tab and under the Grants section, select Submissions

From the Submissions - Incomplete List page, find the Performance Report that you want to work on by using the following filter options. 

A. You can sort the list of submissions by due date. 

B. Or filter by submission name, submission type, organization name, grant #, deadline, and status. 

C. To quickly find an OGH Performance Report, you can type PFR in the Tracking # column filter. 

D. And select the filter icon, and select Contains to narrow the list of submissions for just OGH Performance Reports.

Once you've found your pending report, select Start or Edit from the Options column to begin or continue working on your report.

The Performance Module Form opens in a new window. To review the details of the grant and the helpful resources, expand the headers. 

A. Expand the top section where the Performance Report tracking number is listed, starting with "PFR" to see the Project Period, Reporting Period, Grant Number, Available Date, Submission Status, and Due Date. 

B. Expand the Resources section to view the Awards History. 

C. The Note here shows how to access the Performance Report Reference Document and shortcut back to this Help page.

There are four main sections of the form: 

A. The Performance Report with the indicators. This is where you will respond to the indicators, grouped by domain. 

B. Comments section. This is where you will review any comments provided by HRSA, or you can provide your own comments. This is useful if you have a report that was reviewed by OGH staff and returned to you for changes, as you can review the comments from OGH on what needs to be changed. 

C. PO Attachments. Here you can review any attachments provided by OGH staff.

D. Attachments. This is where you can upload any relevant attachments needed for your performance report.

Completing the Performance Report section. 

  • Each indicator will have a prompt and response section.
  • Note any warning or error messages displayed over each response field.

7.The indicators have different formats of prompts and responses. Here are a few examples of the types of formats you will encounter. 
8.For a Single-Select indicator, you will see the option to Select One from the dropdown list.

For a Multi-Select indicator, the dropdown will say "Number of Items Selected: (0)" indicating that you can select more than one item. 

The dropdown menu will now have checkboxes. Select the checkbox for each item you want to select. 

The items you select will then be added, and you can select the x next to each one if you want to delete it.

For a Textbox indicator, the character limit will be displayed under the textbox. Make sure not to exceed the character limit.

For Number indicators, the system validates the values you enter to ensure they meet specific criteria.

  • This includes checks for whether the number is a whole number or allows decimal precision, as well as enforcing upper and lower limits.
  • If the entered value is invalid, a message will appear at the top of the screen explaining what needs to be corrected.

Note: Some Number indicators may be dependent on other fields. If that is the case, you will see a validation message to guide you in adjusting the numbers accordingly.

Some indicators may have sub-indicators that are only displayed based on certain answers.

  • For example, if this indicator is answered as "N/A," there will be no sub-indicator. If the answer is selected as "No", there will be a subindicator that will be required.

In the Attachments section, upload any relevant documents needed for your Performance Report. 

  • Acceptable Formats include: pdf, doc, docx, xlsx, xls, and csv
  • Select the button Select Files and follow the guidance to upload attachments.

Submitting the Performance Report

To complete and submit the Performance Report, select Save and Submit

  • Any validation messages will be listed at the top of the page for you to review and fix any remaining fields.

Select Confirm to certify that you have the authority to authorize this report and submit it to HRSA.

Requesting a Deadline Extension

In the Top section, make sure to note the Due Date

  • If the due date has passed, some reports may allow you to request a deadline extension.

If the report deadline allows requests for an extension, you may request an extension from the Submissions - Incomplete List page by either:

A. Selecting Request Extension for a report that is already late. 

B. Or select the arrow in the Options column and then select Request Extension
