Outreach and Education

Form Overview

On the Outreach and Education form, the grantee provides details on outreach and education activities provided during the reporting period. The form will display as “Outreach and Education” under the ‘Activity Forms’ section on the left menu.

Outreach and Education refers to activities to inform, generate interest, and provide more in-depth messaging on topics of interest. Outreach can be seen as a way to introduce the topic during brief interactions. Education can be seen as one of those activities that allow messaging and discussion to be tailored to individuals and small groups, as staff respond to questions and address concerns about a topic.

Outreach and Education

The report types are as follows:

  • New Competing Performance Reports (NCPR)
  • Non-Competing Continuation Performance Reports (NCCPR)
  • Project Period End Reports (PPER)

Pre-population

Data will not pre-populate from one report to the next report. The grantee is required to enter the data in each report for this form, if applicable.

Outreach and Education During Reporting Period

In this section, the grantee must answer the following question:

During the reporting period, did your program provide outreach and education?

  • The grantee must Select Yes or No to indicate whether their program provided outreach and education during the reporting period. If the grantee selects Yes, then both Part A and B will be enabled to enter data. If the grantee selects No, then both Part A and B will be disabled, and grantee must click on Save and Continue to move to next form.
  • This is a required question, and the following validation will display above the Yes/No radio buttons: Please provide a response for this field.

Part A. Outreach and Education (excluding web and social media analytics)

In this section, the grantee provides information on outreach and education activities, excluding information on web and social media analytics (captured in Part B), by answering the following questions.

This is a required section if the grantee answers Yes to the question under Outreach and Education During Reporting Period section.

Mechanism of outreach/education

  • In this question, the grantee must select one or more options from the checkboxes that indicate the mechanism used for outreach/education (excluding web and social media analytics) during the reporting period. The grantee must select at least one of the following options:
    • Community/public events
    • Conference presentations
    • Educational materials
    • Webinars
    • Other (specify)

NOTE: If the grantee selects the Other (specify) checkbox, a description is required in the Other Description field. The Other Description field accepts text, numbers, and special characters (not to exceed 1,000 characters).

Subject area(s) of outreach/education:

  • In this question, the grantee must select one or more options from the checkboxes that are closest to the subject areas of their outreach and education activities during the reporting period.
  • If the specific subject area of their outreach and education is not listed, the grantee should select the subject area closest to their subject area. If none of the subject areas are close to the subject area, then the grantee should select None of the above. Refer to the detail sheet at top of the form for definitions. If the grantee selects None of the above, then they will not be able to select any other checkboxes.
  • The grantee must select at least one of the following options:
    • Clinical Care Related (including medical home)
    • Data, Research, Evaluation Methods
    • Emerging Issues (Provide description below)
    • Cultural Responsiveness Related
    • Family Involvement
    • Health Care Workforce Leadership
    • Interdisciplinary Teaming
    • Policy
    • Systems Development/Improvement (including capacity building, planning, and financing)
    • None of the above

NOTE: If the grantee selects the Emerging Issues (Provide description below) checkbox, a description is required in the Emerging Issues Description field. The Emerging Issues Description field accepts text, numbers, and special characters (not to exceed 1,000 characters).

Topics of outreach/education

  • In this question, the grantee selects one or more options from the checkboxes that list the topic of outreach/education activities during the reporting period.
  • If the specific topic area of the outreach and education is not listed, the grantee should select the topic area closest to their topic area. If none of the topic areas are closest to the topic, then the grantee should select None of the above. Refer to Detail Sheet at the top of the form for definitions of topic areas. If the grantee selects None of the above, then they will not be able to select any other checkboxes.

  • The grantee will be able to view the following instruction when the mouse is hovered over the info icon next to the question text:

You may select a subtopic without also selecting the corresponding general topic. For example, you may select Early Childhood - Newborn Screening without selecting Early Childhood - General. In addition, you may select only the general topic if none of the subtopics apply.

Outputs

In this section, the grantee provides information on the number of individuals reached by outreach and education activities in the reporting period for Part A.

Number of individuals reached:

  • The grantee must enter the number of individuals reached by outreach and education activities. This number may be duplicated (i.e., a recipient participates in more than one Outreach/Education activity and is counted more than once), though an unduplicated count is encouraged if possible.
  • This is a required field if the grantee answers Yes to the question under the Outreach and Education During Reporting Period
  • This field accepts integer values between 0-999,999.
  • If the grantee does not know the number of individuals reached by their outreach and education activities in the reporting period, a value of 0 should be entered.

Part B. Web and Social Media Analytics

The grantee may enter the number of web hits, number of unique website visitors, number of social media views, and/or number of unique viewers of social media content for outreach and education materials and resources in the reporting period. This section is optional, and the grantee may answer the following questions only if applicable.

Number of web hits:

  • The grantee may enter the count of web hits.
  • This field accepts integer values between 0-999,999.

 Number of unique website visitors

  • The grantee may enter the count of unique visitors on a website.
  • This field accepts integer values between 0-999,999.

Number of social media views

  • The grantee may enter the count of views of social media content.
  • This field accepts integer values between 0-999,999.

Number of unique viewers of social media content

  • The grantee may enter the count of unique viewers of social media content.
  • This field accepts integer values between 0-999,999.

Comments

In this section, the grantee can enter additional information (not to exceed 5,000 characters). The Comments field accepts text, numbers, and special characters."

The grantee must click on Save or Save and Continue buttons before navigating away from this form to ensure all data entered is saved successfully.

  • The Save button will save the validated information captured in the form and the grantee will remain on the same form.
  • The Save and Continue button will save the validated information and the grantee will navigate to the next form.
  • The Go to Previous Page button will navigate the grantee to the previous form without saving any previously unsaved information.

Form-Level Rules and Validations

  • For New Competing Performance Reports (NCPRs), this form is not applicable.
  • For Non-Competing Continuation Performance Reports (NCCPRs) and Project Period End Reports (PPERs), the grantee must provide data based on the Yes/No selection in the question under Outreach and Education During Reporting Period
  • Part A Outreach and Education is required and will be enabled for data entry only if the user has selected Yes in Outreach and Education During Reporting Period section.
  • Part B Web and Social Media Analytics is an optional section and will be enabled for data entry only if the user has selected Yes in Outreach and Education During Reporting Period section.
  • Data checks and validations for all integer fields.
    • Accepts integer values only.
    • Accepts a maximum of six digits.
    • Does not accept decimal values.
    • Does not accept any text values.
  • If the grantee selected Other (specify) or Emerging Issues checkbox, the corresponding description for that field is required.
    • Data checks and validations for all the description fields such as Other Description and Emerging Issues Description:
      • Fields accept text, numbers, and special characters.
      • Fields accept a maximum of 1,000 characters.
The Comments text box will accept text, numbers, and special characters (not to exceed 5,000 characters).
