Training Form 14

Form Overview

On Training Form 14, the grantee provides details on the percentage of Level I medium-term trainees who report an increase in knowledge and the percentage of Level II medium-term trainees who report an increase in knowledge or skills related to MCH core competencies. The form will display as “Training 14” under the ‘Training and Workforce Forms’ section on the left menu.

The report types are as follows:

  • New Competing Performance Reports (NCPR)
  • Non-Competing Continuation Performance Reports (NCCPR)
  • Project Period End Reports (PPER)

Pre-population

The following data will pre-populate from one report to the next report: 

  •  Data entered into the Annual Performance Objectives section.

Note: Data entered into the NCPR will pre-populate to subsequent NCCPRs and PPERs. Data in this section will also pre-populate between the last approved report created prior to August 1, 2024, and those reports created after.

Section I. Annual Performance Objectives

  • In this section, the grantee must provide objectives for New Competing Performance Reports (NCPRs):
    • Level I – Knowledge (required): Enter an integer from 0–100 for each budget period.
    • Level II – Knowledge (required): Enter an integer from 0–100 for each budget period.
    • Level II – Skills (required): Enter an integer from 0–100 for each budget period.
  • For Non-Competing Continuation Performance Reports (NCCPRs) and Project Period End Reports (PPERs):
    • Pre-population: All budget period objectives will be prepopulated from the previously approved report.
    • Annual Performance Objectives: For current and previous years, annual objective fields will not be editable.
  • For Non-Competing Continuation Performance Reports (NCCPRs) future years, grantees will have the ability to edit/modify the annual objective fields if needed.
  • For Non-Competing Continuation Performance Reports (NCCPRs) and Project Period End Reports (PPERs) the grantee provides data in Section II.
  • A note will display under the blue header Annual Performance Objectives as follows:

For New Competing reports, you must provide objectives in Section I.

For Non-Competing Continuation reports, you may update the objectives for future years in Section I, if needed.

For Non-Competing Continuation and Project Period End reports, you must provide data in Section II.

Section II. Data Collection for Detail Sheet – Medium-Term Trainees Skill and Knowledge

This is a required section for NCCPRs and PPERs, and the grantees must answer the questions in the following sub sections:

  • Level I Medium-Term Trainees – Knowledge
  • Level II Medium-Term Trainees – Knowledge
  • Level II Medium-Term Trainees - Skills
Level I Medium-Term Trainees - Knowledge

This is a required section, and the grantee must answer the following questions:

A. The total number of Level I Medium-Term Trainees (40-149 hours)

  • This is a required field, and the following validation will display on the integer box: Enter an integer: 0 - 999,999

B. The total number of Level I MTT lost to follow-up

  • This is a required field, and the following validation will display on the integer box: Enter an integer: 0 - 999,999

C. The total number of respondents (A-B) = Denominator

  • This is an auto calculated field which is calculated as: Value entered in The total number of Level I Medium-Term Trainees (40-149 hours) minus the value entered in B. The total number of Level I MTT lost to follow-up.

D. Number of respondents reporting increased knowledge (Numerator)

  • This is a required field, and the following validation will display on the integer box: Enter an integer: 0 - 999,999

E. Percentage of respondents reporting increased knowledge

  • This is an auto calculated field which is calculated as: [ Number of respondents reporting increased knowledge (Numerator)]/[C. The total number of respondents (A-B) = Denominator] * 100

Level II Medium-Term Trainees - Knowledge

This is a required section, and the grantee must answer the following questions:

A. The total number of Level II Medium-Term Trainees (150-299 hours)

  • This is a required field, and the following validation will display on the integer box: Enter an integer: 0 - 999,999

B. The total number of Level II MTT lost to follow-up

  • This is a required field, and the following validation will display on the integer box: Enter an integer: 0 - 999,999

C. The total number of respondents (A-B) = Denominator

  • This is an auto calculated field which is calculated as: Value entered in The total number of Level II Medium-Term Trainees (150-299 hours) minus the value entered in B. The total number of Level II MTT lost to follow-up.

D. Number of respondents reporting increased knowledge (Numerator)

  • This is a required field, and the following validation will display on the integer box: Enter an integer: 0 - 999,999

E. Percentage of respondents reporting increased knowledge

  • This is an auto calculated field which is calculated as: [ Number of respondents reporting increased knowledge (Numerator)]/[C. The total number of respondents (A-B) = Denominator] * 100

Level II Medium-Term Trainees - Skills

This is a required section, and the grantee must answer the following questions:

A. The total number of Level II Medium-Term Trainees (150-299 hours)

  • The value entered in The total number of Level II Medium-Term Trainees 150-299) must be equal to the value entered in A. The total number of Level II Medium-Term Trainees 150-299) under Level II Medium-Term Trainees – Knowledge section and the following validation will display on this field:
    • Number of trainees for Level II Medium-Term Trainees - Skills should equal the number of trainees for Level II Medium-Term Trainees - Knowledge

B. The total number of Level II MTT lost to follow-up

  • This is a required field, and the following validation will display on the integer box: Enter an integer: 0 - 999,999

C. The total number of respondents (A-B) = Denominator

  • This is an auto calculated field which is calculated as: Value entered in The total number of Level II Medium-Term Trainees (150-299 hours) minus the value entered in B. The total number of Level II MTT lost to follow-up.

D. Number of respondents reporting increased skills (Numerator)

  • This is a required field, and the following validation will display on the integer box: Enter an integer: 0 - 999,999

E. Percentage of respondents reporting increased knowledge

  • This is an auto calculated field which is calculated as: [ Number of respondents reporting increased skills (Numerator)]/[C. The total number of respondents (A-B) = Denominator] * 100

Section III. Annual Performance Score

This section will be automatically populated based on the information provided in Section II. Data Collection for Detail Sheet – Medium-Term Trainees Skill and Knowledge.

  • A note will display under the blue header Annual Performance Score as follows:

The score is calculated based on the information provided in Section II.

  • A table will display with three columns of auto calculated percentage fields for current reporting period as follows:
    • Level I (%) (Knowledge)  

(Numerator/Denominator * 100)

  • Level II (%) (Knowledge)

Comments

In this section, the grantee can enter additional information (not to exceed 5,000 characters). The Comments field accepts text, numbers, and special characters.

The grantee must click on Save or Save and Continue buttons before navigating away from this form to ensure all data entered is saved successfully.

  • The Save button will save the validated information captured in the form and the grantee will remain on the same form.
  • The Save and Continue button will save the validated information and the grantee will navigate to the next form.
  • The Go to Previous Form button will navigate to the previous form without saving any previously unsaved information.

Form-Level Rules and Validations

  • For New Competing Performance Reports (NCPRs), the grantee must provide targets in Section I.
  • For Non-Competing Continuation Performance Reports (NCCPRs), the grantee may update the targets for future years in Section I, if needed.
  • For Non-Competing Continuation Performance Reports (NCCPRs) and Project Period End Reports (PPERs), the grantee must provide data in Section II.
  • Data checks and validations for the Section I:
    • Accepts integer values between 0 - 100.
    • Does not accept decimal values.
    • Does not accept any text values.
  • The Comments text box accepts text, numbers, and special characters (not to exceed 5,000 characters).
