Form 10 – Project-Developed Measures – Annual Objectives and Performance Data

Form Overview

On Form 10 – Project-Developed Measures – Annual Objectives and Performance Data, the grantee enters the data for the measures that were created in Form 10 – Project-Developed Measures. Note that the Performance Measure title, Numerator, and Denominator fields will be displayed in DGIS in Form 10 – Project-Developed Measures – Annual Objectives and Performance Data exactly as they are defined in Form 10 – Project-Developed Measures.

NOTE: The grantee must complete Form 10 – Project-Developed Measures and click “Save” or “Save and Continue” before completing Form 10 – Project-Developed Measures – Annual Objectives and Performance Data

The new Form 10 will help the grantees develop their own project performance measures to report by collecting and displaying the following:

  • Detail sheets for each project-developed measure.
  • Planned, future year (up to 5 years) Annual Performance Objective targets for each project-developed measure.
  • Annual Performance Indicator values achieved during the reporting period for each project-developed measure.

The form will display as ‘Form 10 – Project-Developed Measures – Annual Objectives and Performance Data’ under the ‘Form 10’ section on the left menu when the grantee starts/edits a DGIS performance report.

The grantee can access the form in two ways:

  • Clicking on Form 10 – Project-Developed Measures – Annual Objectives and Performance Data on the left menu.
  • Clicking on Measure Link to Annual Objectives and Performance Data available for the saved measure on the Form 10 - Project-Developed Measures. Form 10 – Project-Developed Measures – Annual Objectives and Performance Data will open in a separate tab.
Form 10 – Project-Developed Measures – Annual Objectives and Performance Data can only be accessed once the grantee clicks on Save or Save and Continue on the Form 10 - Project-Developed Measures to enable the Measure Link to Annual Objectives and Performance Data and display measures in this form.

Applicable Report Types

The report types are as follows:

  • New Competing Performance Reports (NCPR)
  • Non-Competing Continuation Performance Reports (NCCPR)
  • Project Period End Reports (PPER)

Pre-population

Annual objectives and performance data for Project-Developed Measures created in the NCPR and NCCPRs will pre-populate from one report to the next report. Data will be entered into the NCPR and NCCPRs and pre-populate to subsequent NCCPRs and PPER.

Form 10 – Project-Developed Measures – Annual Objectives and Performance Data will not display the pre-populated data until the grantee clicks on Save on the Form 10 - Project-Developed Measures to enable the Measure Link to Annual Objectives and Performance Data and display measures in this form.

For New Competing Performance Reports (NCPRs):

  • There is no pre-population.

For Non-Competing Continuation Performance Reports (NCCPRs):

  • If the measure is created in NCPR and objectives entered in same report, the following will be the pre-population behavior in subsequent NCCPR:
    • Data Available, Numerator, Denominator, and Data Source will pre-populate from previous NCPR for current reporting period and will be editable.
    • Annual Performance Objectives pre-populates for all reporting periods from previous NCPR if the objectives were entered in the NCPR

For Project Period End Reports (PPERs):

  • Data Available, Numerator, Denominator, and Data Source will pre-populate from previous NCCPR for the past reporting periods.
  • Annual Performance Objectives: All budget periods rows will have the values populated from the previous report. None of the budget periods will be editable for data entry.

Form Sections

When measures are not created or Saved in Form 10 - Project-Developed Measures and the grantee visits the Form 10 - Project-Developed Measures - Annual Objectives and Performance Data from the left menu, the form will display no measures and the following note will display:

  • Form 10 - Annual Objectives and Performance Data page is blank until measures are saved and/or added and saved with no errors on Form 10 - Project-Developed Measures. You MUST click "Save" or "Save and Continue" on Form 10 - Project Developed Measures for this page to populate data entry forms.

Measure <1>: <Measure Title> – Detail Sheet

NOTE: The Measure Number and the Measure Title will be dynamically populated based on the number of measures created and the respective Measure Titles added by the grantee in the Form 10 - Project-Developed Measures

In this section, the grantee may expand the accordion menu to view the following details that were entered and saved in the Form 10 - Project-Developed Measures for the measure selected:

  • Measure Number
  • Performance Measure Title
  • Level
  • Goal
  • Definitions
  • Grantee Data Sources and Issues
  • Significance

Measure <1>: <Measure Title> - Measure Status

NOTE: The Measure Number and the Measure Title will be dynamically populated based on the number of measures created and the respective Measure Titles added by the grantee in the Form 10 - Project-Developed Measures

In this section, the grantee must enter the following information for each measure.

  • Measure Status: This field will display the value Active by default when a measure is created. The grantee may change the Measure Status to Inactive using the dropdown.

    • The grantee can only inactivate a measure in subsequent NCCPRs and PPERs and not in the same report in which the measure is created.
    • If the grantee changes the Measure Status to Inactive in the same report in which the measure is created, the following validation will display on the drop down:
      • Measure status cannot be made Inactive in the same report measure is created.

    • The grantee cannot reactivate a measure that was inactivated in previous reports. The following validation will display on the drop down if the grantee tries to reactivate an inactive measure:
      • Inactive measure cannot be reactivated.

    • The grantee will only be able to reactivate a measure if it is accidentally inactivated in the current report , and will not be able to reactivate that measure in any future report.
    • The grantee will continue to see the Inactivated measures on the form and in the read-only and print view of the report.
    • If the grantee inactivates a measure, all the fields except Measure Link to Annual Objectives and Performance Data on the Form 10 - Project-Developed Measures for the inactive measure will be disabled.

NOTE: After inactivating the measure in Form 10 - Project-Developed Measures - Annual Objectives and Performance Data, the grantee must click on Save or Save and Continue on the Form 10 - Project-Developed Measures as well to reflect the update and the header text for the measure selected in Form 10 - Project-Developed Measures will be updated to following format:

  • Measure <1>: <Measure Title> – Inactive

  • Reason for Inactivation: This field is required if the grantee selects Measure Status as Inactive, and the following validation will display above the radio buttons: Please provide a response for this field.

NOTE: If the grantee selects the Other (specify) option, a description is required in the Comments field. The Comments field accepts text, numbers, and special characters (not to exceed 1,000 characters).

Measure <1>: <Measure Title> - Annual Objective and Performance Data

This is a required section, and the grantee answers the following questions for the applicable reporting periods:

  • Annual Performance Objective: This is a required field, and the grantee establishes Annual Performance Objective for all future reporting periods in the DGIS report in which the measure is created (NCPR or all NCCPRs).
NOTE: Annual Performance Objectives are not NOFO objectives. Instead, they are internal targets set by the grantee to help identify and monitor progress on goals for performance over time.
    • For the New Competing Performance Report (NCPR), all budget periods in the current project period will be displayed AND all Annual Performance Objective fields for these budget periods will be editable for data entry.
    • For Non-Competing Continuation Performance Reports (NCCPRs), all budget periods in the current project period with the values populated from the previous report will be displayed and ONLY future budget periods will be editable for data entry. In other words, the grantee can edit future objectives but cannot edit current or past objectives.
      • The current reporting period will be editable ONLY if the measure is created in the same NCCPR.
    • For the Project Period End Report (PPER), all budget periods in the current project period with the values populated from the previous report will be displayed and none of the budget periods will be editable for data entry.

The field accepts values based on Unit Type selected as follows:

    • Percentage: The field accepts values 0-100 and the following validation will display on the integer box: Enter an integer: 0 – 100
    • Ratio: the field will accept any value between 0 to 1 up to two decimal points. The following validation will display on the integer box: Enter an integer: 0 – 1
    • Scale or Count: the field will accept any value between 0 and the value entered in Unit Number on Form 10 - Project-Developed Measures for that measure. The value entered in Unit Number for Scale or Count is the maximum value for the Unit Type. The following validation will display on the integer box: Enter an integer: 0 – <value entered in Unit Number>.

NOTE: For example, if Unit Type is selected as Count and Unit Number entered is 20 in Form 10 - Project-Developed Measures for that measure, the following validation will display on the integer box: Enter an integer: 0 – 20.

    • Rate: the field will accept any value between 0 to 999,999. The following validation will display on the integer box: Enter an integer: 0 – 999,999
    • Narrative: the Annual Performance Objective will be disabled for all reporting periods.

Note: The screenshot below is an example using the Unit Type of Percentage. The integer values in the validation will be updated based on Unit Type selected.

  • Data Available: This field will display the value Yes by default for the current reporting period and
    • Numerator, Denominator, and Data Source fields of the row will be enabled for data entry if Unit Type is Percentage, Rate and Ratio.
    • Denominator, and Data Source fields of the row will be enabled for data entry if Unit Type is Scale and Count
    • Annual Performance Indicator of the row will be enabled for data entry if Unit Type is Narrative

    • If data is not available for reporting, the grantee may change the Data Available to No using the dropdown. When the grantee selects No, the Numerator, Denominator and Data Source fields of the row will be disabled for data entry.

  • Numerator and Denominator:
    • If the grantee selects Unit Type as Percentage, Rate, or Ratio, the grantee will be required to enter data in Numerator and Denominator, and the following validation will display on the integer box: Enter an integer: 0 – 999,999
      • The following validation will display on the Denominator integer box, if the data entered in Numerator is greater than the data entered in Denominator: Denominator must be greater than or equal to Numerator

  • If the grantee selects Unit Type as Scale or Count,
    • The Denominator field will be disabled.
    • The Numerator field will accept any value between 0 and the value entered in Unit Number on Form 10 - Project-Developed Measures for that measure. The following validation will display on the integer box: Enter an integer: 0 – <value entered in Unit Number>

 NOTE: Unit Type is selected as Count and Unit Number entered is 20 in Form 10 - Project-Developed Measures for that measure, the following validation will display on the integer box: Enter an integer: 0 – 20. The value 20 entered in Unit Number is for illustrative purposes only and will differ in the system depending on the value submitted by the grantee.

    • If the grantee selects Unit Type as Narrative, the Numerator and Denominator fields will be disabled.

  • Annual Performance Indicator: This field will be an auto-calculated field or text field based on the Unit Type selected and the calculations are as follows:
    • If the grantee selects Unit Type as Percentage, the Annual Performance Indicator will be an auto-calculated field with calculation: Numerator/Denominator * 100

    • If the grantee selects Unit Type as Scale or Count, the Annual Performance Indicator will be auto-populated with the value entered in Numerator.

    •  If the grantee selects Unit Type as Rate, the Annual Performance Indicator will be an auto-calculated field with calculation – Numerator/Denominator * [value entered in the Unit Number field on Form 10 - Project-Developed Measures for that measure]

Note: Unit Number entered in Form 10 - Project-Developed Measures for that measure for this report was 10,000. The value 10,000 entered in Unit Number is for illustrative purposes only and will differ in the system depending on the value submitted by the grantee.

    • If the grantee selects Unit Type as Ratio, Annual Performance Indicator will be an auto-calculated field with calculation – Numerator/Denominator.

    • If the grantee selects Unit Type as Narrative, the Annual Performance Indicator will be a text field, and the following validation will display on the integer box: Please provide a response for this field. The field will be editable for the grantee to enter Narrative response (not to exceed 25 characters).

  • Data Source: This is a required field if Data Available is selected as The grantee enters the source(s) of the data used in determining the value of the measure and the time period the data source reflects (not to exceed 250 characters).

Comments Section

The grantee can enter additional information or explain all data estimates in the comment section (not to exceed 5,000 characters).

    • If the data provided was not an estimate, the grantee may leave this field blank.
    • If neither actual data nor an estimate can be provided, the grantee must provide a note in the comment box describing a plan and timeframe for providing the required data.

The grantee must click on Save or Save and Continue buttons before navigating away from this form to ensure all data entered is saved successfully.

  • The Save button will save the validated information captured in the form and grantee will remain on the same form.
    • If the grantee saves the form without entering data for the created measure, an error will display above Save button indicating the error count. If the grantee has not clicked on the fields of the form, the validations will continue to show as warnings and not change to errors when the grantee saves the form.

    • If the grantee has clicked on the fields but has not entered the data, the validations will turn to errors when the grantee saves the form.

  •  The grantee will see the “!” mark preceding the form name on the left menu if the form is saved with errors. The grantee will be required to enter data in all required fields to save the form successfully.

  • The Save and Continue button will save the validated information and grantee will navigate to the next form.
  • The Go to Previous Form button will navigate to the previous form without saving any previously unsaved information.

Form-Level Rules and Validations

  • For New Competing Performance Reports (NCPRs):
    • Data Available will not pre-populate and will be editable for the grantee to enter data for the current budget period. All the future budget periods rows will be disabled.
    • Annual Performance Objectives rows for all budget periods will be editable for the grantee to enter data. The grantee will be required to set these objectives in this report.
    • Numerator and Data Source will be editable for the grantee to enter data for the current budget period and the grantee will be required to enter data if Data Available is selected as Yes. Numerator field will be disabled if Unit Type is selected as Narrative.
    • Denominator will be editable for the grantee to enter data for the current budget period and the grantee will be required to enter data if Data Available is selected as Yes. Denominator field will be disabled for current budget period if Unit Type is selected as Scale, Count, or Narrative.
    • Data Available, Numerator, Denominator, and Data Source will not be editable for future budget periods.
  • For Non-Competing Continuation Performance Reports (NCCPRs):
    • If the measure is created in NCPR and objectives entered in same report, the following will be the behavior in subsequent NCCPR:
      • Data Available, Numerator, Denominator, and Data Source will pre-populate from previous NCPR for current reporting period and will be editable. These fields will be disabled for future reporting periods.
      • Annual Performance Objectives pre-populates for all reporting periods from previous NCPR and ONLY the future reporting periods will be editable for data entry.
      • Data Available will be editable for the current reporting period and default to Yes. This field will be disabled for future reporting periods.
    • If the measure is created in any NCCPR and objectives entered in same report, the following will be the behavior in subsequent NCCPR:
      • The current reporting period for Annual Performance Objectives will be editable if the measure is created in the same NCCPR.
      • Numerator and Data Source will be editable for the grantee to enter data for the current reporting period and the grantee will be required to enter data if Data Available is selected as Yes. Numerator field will be disabled if Unit Type is selected as Narrative.
      • Denominator will be editable for the grantee to enter data for the current project period and the grantee will be required to enter data if Data Available is selected as Yes. Denominator field will be disabled for current budget period if Unit Type is selected as Scale, Count, or Narrative.
      • Data Available, Numerator, Denominator, and Data Source will not be editable for future reporting periods.
  • For Project Period End Reports (PPERs):
    • Annual Performance Objectives: All budget periods rows will have the values populated from the previous report. None of the budget periods will be editable for data entry.
    • Data Available will have the values pre-populated from the previous report for the past reporting periods and it will be editable for the current reporting period for the grantee to enter data.
    • Numerator and Data Source will be editable for the grantee to enter data for the current reporting period and the grantee will be required to enter data if Data Available is selected as Yes. Numerator field will be disabled if Unit Type is selected as Narrative.
    • Denominator will be editable for the grantee to enter data for the current reporting period and the grantee will be required to enter data if Data Available is selected as Yes. Denominator field will be disabled for current reporting period if Unit Type is selected as Scale, Count, or Narrative.
  • For New Competing Performance Reports (NCPRs) and Non-Competing Continuation Performance Reports (NCCPRs), the grantee may create the measures.
  • Data checks and validations for all integer fields:
    • Accepts integer values only.
    • Accepts a maximum of six digits (unless a different maximum value has been noted above).
    • Does not accept decimal values.
    • Does not accept any text values.
  • Data checks and validations for Annual Performance Objective field when Unit Type is Ratio:
    • Accepts numeric values only.
    • Accepts a maximum of one digit.
    • Accepts decimal values up to two decimal points.
    • Does not accept any text values.
  • Data checks and validations for Annual Performance Objective field when Unit Type is Percentage:
    • Accepts integer values only.
    • Accepts a maximum of three digits.
    • Does not accept decimal values and any text values.
  • Data checks and validations for Annual Performance Objective field when Unit Type is Rate, Count or Scale.
    • Accepts integer values only.
    • Accepts a maximum of six digits.
    • Does not accept decimal values and any text values.
  • Data checks and validations for Numerator field when Unit Type is Percentage, Rate, Scale, Count or Ratio:
    • Accepts integer values only.
    • Accepts a maximum of six digits.
    • Does not accept decimal values and any text values.
  • Data checks and validations for Denominator field when Unit Type is Percentage, Rate or Ratio:
    • Accepts integer values only.
    • Accepts a maximum of six digits.
    • Does not accept decimal values and any text values.
  • Data checks and validations for Annual Performance Indicator field when Unit Type is Narrative.
    • Fields accept text, numbers, and special characters.
    • Fields accept a maximum of 25 characters.
  • Data checks and validations for Data Source field:
    • Fields accept text, numbers, and special characters.
    • Fields accept a maximum of 250 characters.
The Comments text box accepts text, numbers, and special characters (not to exceed 5,000 characters).
