• In this section, the grantee must provide objectives for New Competing Performance Reports (NCPRs):
  • 2 Year (%): Enter an integer from 0–100 for each budget period.
  • 5 Year (%): Enter an integer from 0–100 for each budget period.
• For Non-Competing Continuation Performance Reports (NCCPRs) and Project Period End Reports (PPERs):
  • Pre-population: All budget period objectives will be prepopulated from the previously approved report.
  • Annual Performance Objectives: For current and previous years, annual objective fields will not be editable.
• For Non-Competing Continuation Performance Reports (NCCPRs) future years, grantees will have the ability to edit/modify the annual objective fields if needed.

• For Non-Competing Continuation Performance Reports (NCCPRs) and Project Period End Reports (PPERs) the grantee provides data in Section II.

• A note will display under the blue header Annual Performance Objectives as follows:

For New Competing reports, you must provide objectives in Section I.

For Non-Competing Continuation reports, you may update the objectives for future years in Section I, if needed.

For Non-Competing Continuation and Project Period End reports, you must provide data in Section II.

This section is available to the grantees to enter data in NCCPRs and PPERs, and the grantee may answer the following question:

• The notes will display under the blue header as follows:
  Trainees should be tracked based on when they graduate from the undergraduate institution. For example, if a LEAP trainee graduates in 2020, 2-year follow-up should be collected and reported to MCHB in 2022 and 5-year follow-up should be collected and reported in 2025.
  Each LEAP trainee should be counted once.

Does  your LEAP Program have trainees that graduated:

• This is an optional question, and the grantee may select one or more options from the checkboxes. 
  
  • If the grantee selects the 2 years ago checkbox, 2 YEARS AFTER GRADUATING FROM MCH LEAP PROGRAM section will be enabled for the grantee to enter data.
  • If the grantee selects the 5 years ago checkbox, the 2 years ago checkbox would also auto check enabling both 2 YEARS AFTER GRADUATING FROM MCH LEAP PROGRAM and 5 YEARS AFTER GRADUATING FROM MCH LEAP PROGRAM for the grantee to enter data. 

This is a required section if the grantee answers the 2 years ago checkbox in response to the Does your LEAP Program have trainees that graduated question above.

A. The total number of LEAP Trainees that graduated 2 years ago

• This is a required field, and the following validation will display on the integer box: Enter an integer: 0 - 999,999
• This value should be consistent across Training forms 7, 8, and 9. If the value doesn’t match with other forms, the following validation will display: This value should be consistent across Training Measures 7, 8, 9

B. The total number of graduates lost to follow-up

• This is a required field, and the following validation will display on the integer box: Enter an integer: 0 - 999,999

C. The total number of respondents (A-B) = Denominator

• This is an auto calculated field which is calculated as: Value entered in A. The total number of LEAP Trainees that graduated 2 years ago minus the value entered in B. The total number of graduates lost to follow-up.

D. Specify the number of respondents that are enrolled in or have completed the following graduate programs:

• This is a required question, and the grantee must provide the total number of respondents under following graduate programs:
  • Medicine (e.g. Pediatric, Ob/Gyn, Primary Care): This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Public Health: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Nutrition: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Social work: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Nursing: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Pediatric dentistry: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Psychology: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Pediatric occupational/physical therapy: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Speech language pathology: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Other MCH-related health profession description: This is an optional question, and the text field accepts text, numbers, and special characters (not to exceed 100 characters).
  • Other MCH-related health profession: This is a required question if a value is entered in Other MCH-related health profession description field, and the following validation will display: Enter an integer from 0–999,999.
  • Total number of respondents that are enrolled in or have completed graduate programs preparing them work with the MCH population (Numerator): This is an auto calculated field which is calculated as a sum of values entered in all the above fields.

E. Percent of respondents that are enrolled in or have completed graduate programs preparing them to work with the MCH population (%)

• This is an auto calculated field which is calculated as: [Total number of respondents that are enrolled in or have completed graduate programs preparing them work with the MCH population (Numerator)]/[ The total number of respondents (A-B) = Denominator] * 100

F. Number of LEAP trainees who indicate MCH LEAP Training Program helped in admission to a graduate program (Numerator)

 G. Percent of LEAP trainees who indicate MCH LEAP Training Program helped in admission to a graduate program (%)

 I. Percent of LEAP trainees who indicate MCH LEAP Training Program helped in being successful in a graduate program (%)

In this section, the grantee can enter additional information (not to exceed 5,000 characters). The Comments field accepts text, numbers, and special characters.

The grantee must click on Save or Save and Continue buttons before navigating away from this form to ensure all data entered is saved successfully.

• The Save button will save the validated information captured in the form and the grantee will remain on the same form.
• The Save and Continue button will save the validated information and the grantee will navigate to the next form.
• The Go to Previous Form button will navigate to the previous form without saving any previously unsaved information.
