MCH LEAP Training Program - Trainee Information Form

Form Overview

On the MCH LEAP Training Program - Trainee Information Form, the grantee provides aggregate data on medium-term and long-term LEAP trainees who are participating in the LEAP training program in the reporting period. The grantees report aggregate data on this form based on trainee self-report of data elements. The form will display as “MCH LEAP Training Program - Trainee Information Form” under the ‘Training and Workforce Forms’ section on the left menu.

 The report types are as follows:

  • New Competing Performance Reports (NCPR)
  • Non-Competing Continuation Performance Reports (NCCPR)
  • Project Period End Reports (PPER)
NOTE: In NCPRs, LEAP form will be available in read only mode, and all fields of the form will be blank and disabled. The grantee is required to click on Save or Save and Continue button on this form to submit the report successfully.

Pre-population

Data will not pre-populate from one report to the next report. The grantee is required to enter the data in each report for this form, if applicable.

In this section, the grantee must answer the following question:

Total Number of LEAP Trainees

  • This is a required question, and the grantee must enter the total number of during the 12-month reporting period.
  • The following validation will display: Enter an integer: 0 - 999,999.

Ethnicity

This section is required when the grantee provides a value greater than 0 in Total Number of LEAP Trainees question under LEAP trainees participating in the LEAP training program during the 12-month reporting period section above

Number of LEAP trainees who identify as:

  • Hispanic or Latino: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Not Hispanic or Latino: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Choose not to disclose/Unrecorded: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Total: This is an auto calculated field with sum of values entered in each Ethnicity category, and the Total value should be equal to the Total Number of LEAP Trainees. The following validation will display above the Total field:

Race

This section is required when

Number of LEAP trainees who identify as:

  • American Indian or Alaska Native: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Asian: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Black or African American: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Native Hawaiian or Pacific Islander: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • White: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • More than One Race: This is a required question, and the following validation will display: Enter an integer from 0–999,999.

In this section, all questions are required if the grantee provides a value greater than 0 in Total Number of LEAP Trainees question under LEAP trainees participating in the LEAP training program during the 12-month reporting period section above and the grantee must answer the following questions:

  • 15-19: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • 20-24: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • 25-29: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • 30-34: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • 35 and older: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Total: This section will be automatically populated, and this value must match the Total Number of LEAP Trainees.

In this section, all questions are required if the grantee provides a value greater than 0 in Total Number of LEAP Trainees question under LEAP trainees participating in the LEAP training program during the 12-month reporting period section above and the grantee must answer the following questions:

  • Part-time: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Full-time: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Unrecorded: This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • Total:  This is an auto calculated field with sum of values entered in Part-time and Full-time and Unrecorded categories and the Total value should be equal to the Total Number of LEAP Trainees. The following validation will display above the Total field:

Number of LEAP trainees who:

In this section, all questions are required if the grantee provides a value greater than 0 in Total Number of LEAP Trainees question under LEAP trainees participating in the LEAP training program during the 12-month reporting period section above and the grantee must answer the following questions:

  • Are first-generation college students (First-generation college students are students who enrolled in postsecondary education and whose parents do not have any postsecondary education experience.):
  • This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • The value entered in this question should be less than or equal to the Total Number of LEAP Trainees. The following validation will display above the Total field if this criterion is not met:

Value provided must be less than or equal to Total Number of LEAP trainees

  • Work full-time (>35 hours/week) while enrolled in college (Includes LEAP trainees who have worked full-time (>35 hours/week) at any point during the 12-month reporting period.):
  • This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • The value entered in this question should be less than or equal to the Total Number of LEAP Trainees. The following validation will display above the Total field if this criterion is not met:

Value provided must be less than or equal to Total Number of LEAP trainees

  • Have a dependent(s) other than spouse:
  • This is a required question, and the following validation will display: Enter an integer from 0–999,999.
  • The value entered in this question should be less than or equal to the Total Number of LEAP Trainees. The following validation will display above the Total field if this criterion is not met:

Value provided must be less than or equal to Total Number of LEAP trainees

Comments

In this section, the grantee can enter additional information (not to exceed 5,000 characters). The Comments field accepts text, numbers, and special characters.

The grantee must click on Save or Save and Continue buttons before navigating away from this tab or form to ensure all data entered is saved successfully. If the grantee navigates to the previous tab without clicking on Save, data will not be lost, and the grantee can perform the save action in previous tab. If the grantee navigates to a different form without clicking on Save, data entered in the tab will be lost.

  • The Previous Tab button will save the validated information and the grantee will navigate to the previous tab.
  • The Save button will save the validated information captured in the form and the grantee will remain on the same form.
  • The Save and Continue button will save the validated information and the grantee will navigate to the next form.
  • The Go to Previous Form button will navigate to the previous form without saving any previously unsaved information.

Form-Level Rules and Validations

  • For New Competing Performance Reports (NCPRs), this form is not applicable and will be available in read only view.
  • For Non-Competing Continuation Performance Reports (NCCPRs) and Project Period End Reports (PPERs), the grantee must provide data for all the sections.
  • Data checks and validations for all integer fields:
    • Accepts integer values only.
    • Accepts a maximum of six digits.
    • Does not accept decimal values.
    • Does not accept any text values.
The Comments text box will accept text, numbers, and special characters (not to exceed 5,000 characters).
