Partnerships and Collaboration

Form Overview

On the Partnerships and Collaboration form, the grantee provides details on partnerships and collaboration activities undertaken during the reporting period.

Partnerships and Collaboration refers to activities that build and strengthen connections between organizations and individuals with similar interests, missions, and activities to allow for information sharing, learning, and capacity building across organizations/individuals. These activities include creation or strengthening of relevant organizational relationships that serve to expand the capacity and reach of a program in meeting the needs of its MCH population. Partnerships and collaborations are intended to be mutually beneficial relationships for all parties involved. Programs that build partnerships and collaboration between organizations, but themselves are not active in or beneficiaries of the partnerships (for example, a TA center that sets up a peer-to-peer network but does NOT participate as a recipient or beneficiary), should not complete this form.

The form will display as ‘Partnerships and Collaboration’ under the ‘Activity Forms’ section on the left menu when the grantee starts/edits a DGIS performance report.

 The report types are as follows:

  • New Competing Performance Reports (NCPR)
  • Non-Competing Continuation Performance Reports (NCCPR)
  • Project Period End Reports (PPER)

Pre-population

Data will not pre-populate from one report to the next report as this is an activity-based form and activities could change year over year. The grantee is required to enter the data in each report for this form, if applicable.

Partnerships and Collaboration During Reporting Period  

In this section, the grantee answers the following question with Yes/No radio buttons:

During the reporting period, did your program engage in or support partnerships and collaborations to expand capacity and reach to meet the needs of the program’s MCH population?

  • If the grantee selects Yes, Part A will be enabled for the grantee to enter data.
  • If grantee selects No, Part A will be disabled, and the grantee must click on Save and Continue to move to the next form.
  • This is a required question, and the following validation will display above the radio buttons:

Part A. Partnerships and Collaboration

In this section, the grantee answers the following question:

  • This is a required question, and the checkboxes will enable for data entry if the grantee answers Yes to the question under the Partnerships and Collaboration During Reporting Period section and the following validation will display above the checkboxes:

  • The grantee must select one or more options from the checkboxes that indicate various purposes. 

NOTE: If the grantee selects the Other (specify) checkbox, a description is required in the Other Description field. The Other Description field accepts text, numbers, and special characters (not to exceed 1,000 characters).

Outputs: Types and numbers of partnerships and collaborations in reporting period, by partner/collaborator category

In this section, the grantee selects all the types of partnership/collaboration that apply and reports the number of partnerships/collaborations in the reporting period.

  • A note will display under the grey header as follows: 

For each applicable partner/collaborator category, select all the types of partnership/collaboration that apply, and report the number of partnerships/collaborations in the reporting period. The number of partnerships/collaborations should be an unduplicated count. Partners/Collaborators can be organizations or individuals.

  • This is a required question, and the checkboxes will enable for data entry if the grantee answers Yes to the question under the Partnerships and Collaboration During Reporting Period section and the following validation will display above the checkboxes:

NOTE: If the grantee selects the Other Partner/Collaborator (specify) checkbox, a description is required in the Other Partner/Collaborator Description field and the following validation will display above the data entry box: Provide a description for "Other Partner/Collaborator Description". The Other Partner/Collaborator Description field accepts text, numbers, and special characters (not to exceed 1,000 characters).

Partnership/Collaboration Details In The Reporting Period

  • This table is required if the grantee answers Yes to the question under the Partnerships and Collaboration During Reporting Period
  • The rows under this table will populate based on the categories selected by the grantee to the question Select all applicable partner/collaborator categories.
  • The table will have following four columns:
  • Partner/Collaborator Category: This column will list down all the categories selected by the grantee to the question Select all applicable partner/collaborator categories.
  • Number of partnerships/collaborations for the partner/collaborator category in the reporting period: The grantee enters an integer count of partnership/collaborations for each partner/collaborator category in the reporting period. This is a required field for each row added to this table and validation message display on the field: Enter an integer: 1 – 999,999.
  • Type of partnership/collaboration (select all that apply): This is a required field for each row added to this table and the following validation message will display on the field: Please provide a response for this field. If all types are applicable, then the grantee can select Check All to select all types in the dropdown. The selected types will be displayed under the dropdown. The grantee selects one or more applicable types of partnership/collaboration from the following dropdown values for each row and can click on x to remove the selections.
    • Information-sharing networks
    • Memoranda of understanding or other written agreements
    • Peer-to-peer learning
    • Provider-to-provider consultations
    • Referral and care coordination networks
    • Shared resources (for example, funding, staff, etc.)
    • Working groups or committees (including advisory boards, steering committees)
    • Other (specify)
  • Other Description (Required if you selected ‘Other’): This field is required if the grantee has selected Other (specify) to Type of partnership/collaboration and validation will display on the field: Provide a description for ‘Other’. The Other Description field accepts text, numbers, and special characters (not to exceed 250 characters).

NOTE: Click Save to ensure the information is not lost due to inactivity on the form.

Comments

In this section, the grantee can enter additional information (not to exceed 5,000 characters). The Comments field accepts text, numbers, and special characters.

The grantee must click on Save or Save and Continue buttons before navigating away from this form to ensure all data entered is saved successfully.
  • The Save button will save the validated information captured in the form and the grantee will remain on the same form.
  • The Save and Continue button will save the validated information and the grantee will navigate to the next form.
  • The Go to Previous Form button will navigate to the previous form without saving any previously unsaved information.

Form-Level Rules and Validations

  • For New Competing Performance Reports (NCPRs), this form is not applicable.
  • For Non-Competing Continuation Performance Reports (NCCPRs) and Project Period End Reports (PPERs), the grantee must provide data based on the Yes/No selection to the question under Partnerships and Collaboration During Reporting Period
  • Part Partnerships and Collaboration is a required section and will be enabled for data entry only if the user has selected Yes to the question under Partnerships and Collaboration During Reporting Period section.
  • Data checks and validations for integer fields:
    • Accepts integer values between 1 – 999,999. 
    • Does not accept decimal values. 
    • Does not accept any text values.  
  • The Comments text box accepts text, numbers and special characters (not to exceed 5,000 characters).
